About This Role

Provide skilled nursing care to patients in their homes across the Greater Houston area. Our nurses are the heart of our clinical team, delivering compassionate, evidence-based care that helps clients maintain their independence and quality of life.

Responsibilities

Conduct comprehensive patient assessments and develop individualized care plans
Administer medications, wound care, IV therapy, and other skilled nursing services
Educate patients and families on disease management and care techniques
Collaborate with physicians and interdisciplinary team members
Maintain accurate clinical documentation in compliance with agency standards

Qualifications

Current Texas RN or LVN license in good standing
Minimum 1 year of clinical experience; home health experience preferred
CPR/BLS certification required
Valid Texas driver's license and reliable transportation
Strong clinical assessment and communication skills
